Paul's Biography: Paul Scharre is a Senior Fellow and Director of the Technology and National Security Program at the Center for a New American Security. He is author of Army of None: Autonomous Weapons and the Future of War. Mr. Scharre formerly worked in the Office of the Secretary of Defense (OSD) where he played a leading role in establishing policies on emerging weapons technologies. He led the working group that drafted DOD Directive 3000.09, establishing DOD’s policy on autonomy in weapon systems. He is a former infantryman in the Army’s 75th Ranger Regiment and completed multiple tours to Iraq and Afghanistan.

 

Radha's Biography: Radha Iyengar is the head of Product Policy Research at Facebook and an adjunct economist at the RAND Corporation. Previously, she served in senior staff positions at the White House National Security Council, Department of Defense, and Department of Energy. Over the course of her government service, she was instrumental in executive actions on sexual assault and suicide prevention, budget and policy related to nuclear and energy infrastructure security and resilience, and security assistance and counterterrorism efforts in the the Middle East and North Africa. Her research has covered empirical evaluations of policies aimed at reducing violence including criminal violence, sexual assault, terrorist behavior, and sexual and intimate partner violence. 

 

Jeremy's Biography: Jeremy is a Professor of Political Science and Senior Fellow at the Freeman Spogli Institute for International Studies. He is also a non-resident fellow at the Center for Global Development in Washington, D.C. His research focuses on civil wars and political violence; ethnic politics and the political economy of development; and democracy, accountability, and political change. He is the author of Inside Rebellion: The Politics of Insurgent Violence (Cambridge University Press), which received the William Riker Prize for the best book on political economy. He is also the co-author of Coethnicity: Diversity and the Dilemmas of Collective Action (Russell Sage Foundation), which received the Gregory Luebbert Award for the best book in comparative politics. He has published articles in the American Political Science Review, American Journal of Political Science, Annual Review of Political Science, Journal of Conflict Resolution, Foreign Affairs, Foreign Policy, Journal of Democracy, World Policy Journal, and the SAIS Review.

Most studies that look at whether democracy improves global health rely on measurements of life expectancy at birth and infant mortality rates. Yet those measures disproportionately reflect progress on infectious diseases — such as malaria, diarrheal illnesses and pneumonia — which relies heavily on foreign aid.

A new study led by Stanford Health Policy's Tara Templin and the Council on Foreign Relations suggests that a better way to measure the role of democracy in public health is to examine the causes of adult mortality, such as noncommunicable diseases, HIV, cardiovascular disease and transportation injuries. Little international assistance targets these noncommunicable diseases. 

When the researchers measured improvements in those particular areas of public health, the results proved dramatic.

“The results of this study suggest that elections and the health of the people are increasingly inseparable,” the authors wrote.

A paper describing the findings was published today in The Lancet. Templin, a graduate student in the Department of Health Research and Policy, shares lead authorship with Thomas Bollyky, JD, director of the Global Health Program at the Council on Foreign Relations.

“Democratic institutions and processes, and particularly free and fair elections, can be an important catalyst for improving population health, with the largest health gains possible for cardiovascular and other noncommunicable diseases,” the authors wrote.

Templin said the study brings new data to the question of how governance and health inform global health policy debates, particularly as global health funding stagnates.

“As more cases of cardiovascular diseases, diabetes and cancers occur in low- and middle-income countries, there will be a need for greater health-care infrastructure and resources to provide chronic care that weren’t as critical in providing childhood vaccines or acute care,” Templin said.

Free and fair elections for better health

In 2016, the four mortality causes most ameliorated by democracy — cardiovascular disease, tuberculosis, transportation injuries and other noncommunicable diseases — were responsible for 25 percent of total death and disability in people younger than 70 in low- and middle-income countries. That same year, cardiovascular diseases accounted for 14 million deaths in those countries, 42 percent of which occurred in individuals younger than 70.

Over the past 20 years, the increase in democratic experience reduced mortality in these countries from cardiovascular disease, other noncommunicable diseases and tuberculosis between 8-10 percent, the authors wrote.

“Free and fair elections appear important for improving adult health and noncommunicable disease outcomes, most likely by increasing government accountability and responsiveness,” the study said.

The researchers used data from the Global Burden of Diseases, Injuries, and Risk Factors StudyV-Dem; and Financing Global Health databases. The data cover 170 countries from 1970 to 2015.

What Templin and her co-authors found was democracy was associated with better noncommunicable disease outcomes. They hypothesize that democracies may give higher priority to health-care investments.

What Templin and her co-authors found was democracy was associated with better noncommunicable disease outcomes. They hypothesize that democracies may give higher priority to health-care investments.

Foreign aid often misdirected

And yet, this connection between fair elections and global health is little understood.

“Democratic government has not been a driving force in global health,” the researchers wrote.  “Many of the countries that have had the greatest improvements in life expectancy and child mortality over the past 15 years are electoral autocracies that achieved their health successes with the heavy contribution of foreign aid.”

They note that Ethiopia, Myanmar, Rwanda and Uganda all extended their life expectancy by 10 years or more between 1996 and 2016. The governments of these countries were elected, however, in multiparty elections designed so the opposition could only lose, making them among the least democratic nations in the world.

Yet these nations were among the top two-dozen recipients of foreign assistance for health.

Only 2 percent of the total development assistance for health in 2016 was devoted to noncommunicable diseases, which was the cause of 58 percent of the death and disability in low-income and middle-income countries that same year, the researchers found.

“Although many bilateral aid agencies emphasize the importance of democratic governance in their policy statements,” the authors wrote, “most studies of development assistance have found no correlation between foreign aid and democratic governance and, in some instance, a negative correlation.”

Autocracies such as Cuba and China, known for providing good health care at low cost, have not always been as successful when their populations’ health needs shifted to treating and preventing noncommunicable diseases. A 2017 assessment, for example, found that true life expectancy in China was lower than its expected life expectancy at birth from 1980 to 2000 and has only improved over the past decade with increased government health spending. In Cuba, the degree to which its observed life expectancy has exceeded expectations has decreased, from four-to-seven years higher than expected in 1970 to three-to-five years higher than expected in 2016.

“There is good reason to believe that the role that democracy plays in child health and infectious diseases may not be generalizable to the diseases that disproportionately affect adults,” Bollyky said. Cardiovascular diseases, cancers and other noncommunicable diseases, according to Bollyky, are largely chronic, costlier to treat than most infectious diseases, and require more health care infrastructure and skilled medical personnel.  

The researchers hypothesize that democracy improves population health because:

  1. When enforced through regular, free and fair elections, democracies should have a greater incentive than autocracies to provide health-promoting resources and services to a larger proportion of the population;
  2. Democracies are more open to feedback from a broader range of interest groups, more protective of media freedom and might be more willing to use that feedback to improve their public health programs;
  3. Autocracies reduce political competition and access to information, which might deter constituent feedback and responsive governance.

Various studies have concluded that democratic rule is better for population health, but almost all of them have focused on infant and child mortality or life expectancy at birth.

Over the past 20 years, the average country’s increase in democracy reduced mortality from cardiovascular disease by roughly 10 percent, the authors wrote. They estimate that more than 16 million cardiovascular deaths may have been averted due to an increase in democracy globally from 1995 to 2015. They also found improvements in other health burdens in the countries where democracy has taken hold: an 8.9 percent reduction in deaths from tuberculosis, a 9.5 percent drop in deaths from transportation injuries and a 9.1 percent mortality reduction in other noncommunicable disease, such as congenital heart disease and congenital birth defects.

“This study suggests that democratic governance and its promotion, along with other government accountability measures, might further enhance efforts to improve population health,” the study said. “Pretending otherwise is akin to believing that the solution to a nation’s crumbling roads and infrastructure is just a technical schematic and cheaper materials.”

The other researchers who contributed to the study are Matthew CohenDiana SchoderJoseph Dieleman and Simon Wigley, from CFR, the University of Washington-Seattle and Bilkent University in Turkey, respectively.

Funding for the research came from Bloomberg Philanthropies and the Bill & Melinda Gates Foundation. Stanford’s Department of Health Research and Policy also supported the work.

From October 22–23, 2018, the U.S.-Asia Security Initiative (USASI) at Stanford University, in conjunction with the Institute for China-U.S. People-to-People Exchange at Peking University and the American Academy of Arts and Sciences (AAAS), gathered scholars and policy practitioners at the Stanford Center at Peking University to participate in the “Civil Wars, Intrastate Violence, and International Responses” workshop. The workshop was an extension of a project examining the threats posed by intrastate warfare launched in 2015 and led by AAAS and Stanford University’s Freeman Spogli Institute for International Studies. The goal of this workshop was to facilitate frank discussions exposing participants to a wide range of views on intrastate violence and international responses.

The workshop was divided into sessions that assessed trends in intrastate violence since the end of the Cold War, examined the threats to international security posed by civil wars and intrastate violence, and evaluated international responses, including an analysis of the limits of intervention and a discussion of policy recommendations. Participants also had an opportunity to make closing comments and recommendations for future research.

This report provides an executive summary and summaries of the workshop sessions on a non-attribution basis.

Abstract: Multilateral conferences are the bread-and-butter of international politics. In such settings, countries may pursue their interests individually, but most of the time they prefer to act through coalitions. Such coalitions are overlapping, creating a network structure. States build and utilize networks to get agenda items pushed through or to block unfavorable ones. While sometimes they are formed on the basis of formal institutions (such as the NAM or the EU), frequently their membership is based on either ad hoc cooperation, or existing informal bodies (such as the NSG, New Agenda Coalition, or Zangger Committee). The attention to such networks is, however, still in its infancy. This paper looks at how state networks within one of the most important recurring diplomatic conferences – the quinquennial NPT Review Conference – develop and transform over time. By doing so, the paper maps the existing networks, and explains their transformation as an instrument of global governance.

 

Speaker Bio: Michal Onderco is a Junior Faculty Fellow at CISAC (2018-2019), and his research focuses on politics of multilateral nuclear diplomacy. His current project tries to understand how states build coalitions in multilateral diplomacy, and why are some coalitions more successful than others.

Michal is currently on leave from Erasmus University Rotterdam, where he is Assistant Professor of International Relations. Previously, he was a Max Weber Fellow at the European University Institute, Fulbright Visiting Researcher at Columbia University in New York, and a short-term Stanton Fellow at Fundação Getúlio Vargas in São Paulo. He received his LLM in Law and Politics of International Security and PhD in Political Science from Vrije Universiteit Amsterdam. His earlier work was published in International Studies Quarterly, European Journal of Political Research, Cooperation & Conflict, The Nonproliferation Review, and European Political Science Review.

Abstract: Materials used in key components of nuclear power reactors, such as the fuel cladding and the pressure vessel, provide shields for the release of highly radioactive isotopes generated in the nuclear fuel to the environment, thus their reliability is an important issue in the safety evaluation.  The accident that occurred at the Fukushima Daiichi in 2011 demonstrated that materials that are considered reliable during normal operating conditions will fail in an extreme accident condition. Subsequently, there has been an international effort on developing materials for Accident Tolerant Fuels (ATF). In addition, the development of new generation of nuclear reactors also calls for new materials that may withstand higher temperatures, higher radiation doses and with better performance under severe corrosive conditions.  This talk will outline the challenges and status for such developments using recent data from the authors’ own research group as examples.  Also, since China is building the most nuclear reactors now and “a nuclear accident anywhere of the world will be an accident of everywhere of the world”, the importance and challenges of collaborating with the Chinese in this area will be discussed. 

Bio: Dr. Lumin Wang is a professor of nuclear engineering, and materials science & engineering at the University of Michigan-Ann Arbor (UM). He came to the US from China in 1982, and received his MS and PhD degrees in Material Science from the University of Wisconsin-Madison in 1984 and 1988, respectively. He worked as a post-doctoral fellow at Argonne National Laboratory and a research scientist at the University of New Mexico before joining the faculty of UM in 1997.  His research has focused on the study of radiation effects of materials using ion beams and transmission electron microscopy. He served as the director of Electron Microbeam Analysis Laboratory, a campus-wide material characterization center at UM between 2005 and 2010. Dr. Wang has published more than 400 papers in research journals and delivered more than 100 invited talks internationally. He has been a member of the International Committee of the American Nuclear Society and an adjunct chair professor of Xiamen University of China since 2011. He has taken more than 100 UM students to China to observe the construction of nuclear reactors during the last 8 summers.

Chris Painter is a globally recognized leader and expert on cyber policy, Cyber Diplomacy and combatting cybercrime.  He has been on the vanguard of US and international cyber issues for over twenty-five years—first as a leading federal prosecutor of some of the most high-profile cybercrime cases in the country, then as a senior official at the Department of Justice, the FBI, the National Security Council and finally at the State Department as the world’s first top cyber diplomat.  He has helped drive, initiated or been involved in most every major US cyber policy for over a decade and has created innovative new organizations and approaches to deal with threats and take advantage of opportunities in cyberspace.  He was named the Bartels World Affairs Fellow for 2017-18 by Cornell University, awarded the Order of the Rising Sun in 2018 by the Government of Japan for promoting Japan-US Cyber collaboration, and awarded the Order of Terra Mariana in 2020 by the President of Estonia.  Among other things, he currently serves as President of the Global Forum on Cyber Expertise Foundation, on the Board of the Center for Internet Security and on a Public Sector Advisory Committee for Palo Alto Networks.  He is also an Associate Fellow with Chatham House and is a Commissioner on the Global Commission for the Stability of Cyberspace.

In his most recent government role as the nation’s top cyber diplomat, Mr. Painter coordinated and led the United States’ diplomatic efforts to advance an open, interoperable, secure and reliable Internet and information infrastructure.  The pioneering office that Mr. Painter established — the Office of the Coordinator for Cyber Issues — was the first high-level position and office dedicated to advancing the diplomatic aspects cyber issues ranging from national security to human rights matters anywhere in the world.   His efforts helped create a new area of foreign policy focus that included promoting norms of responsible state behavior and cyber stability, preventing cyber conflict, enhancing deterrence, advancing cybersecurity, fighting cybercrime, promoting multi-stakeholder Internet governance and advancing Internet freedom.  Mr. Painter was instrumental in negotiating a landmark agreement regarding the theft of intellectual property with China, negotiating a comprehensive cyber cooperation agreement with India, using diplomatic and other tools to combat high-profile cyber-attacks and intrusions, and launching first of their kind “whole of government” cyber dialogues and capacity building programs with dozens of countries in Europe, Asia, the Americas, the Middle East and Africa.  He and his team also spearheaded the promotion of an international framework of cyber stability that includes building a consensus around norms of acceptable behavior and getting agreement on transparency and confidence-building measures designed to reduce the risk of miscalculation that could inadvertently lead to cyber warfare.

From 2009-2011, Mr. Painter served in the White House as the first Senior Director for Cyber Policy and Acting Cyber Coordinator in the National Security Council, where he   advised the President, the National Security Advisor, and the Homeland Security Advisor on a wide range of cyber issues. He was a senior member of the team that conducted the President's Cyberspace Policy Review in 2009 and he subsequently helped create and then structure a new directorate in the National Security Council devoted to these issues.  He also proposed and then coordinated the development of the President’s 2011 International Strategy for Cyberspace — the first such strategy by any country.

Mr. Painter began his federal career as an Assistant US Attorney in Los Angeles where he led some of the highest profile and significant cybercrime prosecutions in the country, including the prosecution of notorious computer hacker Kevin Mitnick, the first two Internet stock manipulation cases in the country, and one of the first high profile distributed denial of service case that knocked many prominent Internet companies off-line.  He subsequently helped lead the case and policy efforts of the Computer Crime and Intellectual Property Section in the US Department of Justice where he supervised national security and cybercrime matters, developed international networks to advance the fight against cybercrime and chaired the cutting edge G8 High Tech Crime Subgroup from 2002-2012.   He also served, for a short time, as Deputy Assistant Director of the FBI’s Cyber Division focusing on a range of national security, cybercrime, international and industry issues.  Prior to joining the US government, Mr. Painter was an attorney at the law firm of Arnold and Porter where he specialized in securities litigation, communications law, international trade and pro bono matters.

Mr. Painter has been a frequent media spokesperson and presenter on cyber issues around the globe, testified on numerous occasions before Congress and published articles on deterrence, diplomacy and other cyber issues.  He is also the recipient of the RSA Award for Excellence in the Field of Public Policy (2016), the CES Government Technology Leadership Award (2018), the Attorney General’s Award for Exceptional Service, the Intelligence Community Legal Award and has been named to the “Federal 100” list, among other honors.  He is a Distinguished Non-resident Fellow at the Australian Security Policy Institute and was a Principle Visiting Fellow at the Hague Center for Strategic Studies.  He is a graduate of Stanford Law School and Cornell University and clerked for US 9th Circuit Judge Betty Fletcher.

Stephen Stedman is a Freeman Spogli senior fellow at the Center on Democracy, Development, and the Rule of Law and FSI, an affiliated faculty member at CISAC, and professor of political science (by courtesy) at Stanford University. 

In 2011-12 Professor Stedman served as the Director for the Global Commission on Elections, Democracy, and Security, a body of eminent persons tasked with developing recommendations on promoting and protecting the integrity of elections and international electoral assistance. The Commission is a joint project of the Kofi Annan Foundation and International IDEA, an intergovernmental organization that works on international democracy and electoral assistance. In 2003-04 Professor Stedman was Research Director of the United Nations High-level Panel on Threats, Challenges and Change and was a principal drafter of the Panel’s report, A More Secure World: Our Shared Responsibility. In 2005 he served as Assistant Secretary-General and Special Advisor to the Secretary- General of the United Nations, with responsibility for working with governments to adopt the Panel’s recommendations for strengthening collective security and for implementing changes within the United Nations Secretariat, including the creation of a Peacebuilding Support Office, a Counter Terrorism Task Force, and a Policy Committee to act as a cabinet to the Secretary-General.  His most recent book, with Bruce Jones and Carlos Pascual, is Power and Responsibility: Creating International Order in an Era of Transnational Threats (Washington DC: Brookings Institution, 2009).

Didi Kuo is a Center Fellow at the Freeman Spogli Institute for International Studies (FSI) at Stanford University. She is a scholar of comparative politics with a focus on democratization, corruption and clientelism, political parties and institutions, and political reform. She is the author of The Great Retreat: How Political Parties Should Behave and Why They Don’t (Oxford University Press, forthcoming) and Clientelism, Capitalism, and Democracy: the rise of programmatic politics in the United States and Britain (Cambridge University Press, 2018).

She has been at Stanford since 2013 as the manager of the Program on American Democracy in Comparative Perspective and is co-director of the Fisher Family Honors Program at CDDRL. She was an Eric and Wendy Schmidt Fellow at New America and is a non-resident fellow with the Carnegie Endowment for International Peace. She received a PhD in political science from Harvard University, an MSc in Economic and Social History from Oxford University, where she studied as a Marshall Scholar, and a BA from Emory University.
